Scheduling An Appointment
With a written referral order from your physician, call central scheduling at 860.271.4848, to schedule your initial evaluation.
How do I make an appointment?
It's very easy, you just need to call our Central Scheduling office to schedule your initial evaluation by dialing 860.271.4848. The fax number for our Central Scheduling office is 860.271-4801. Our schedulers will arrange your initial evaluation at one of our 3 convenient locations. They will collect insurance information, so be sure to have your insurance card handy. They will also verify your insurance over the phone and they can take a co-pay over the phone if you have one.
Do I need a doctor's referral?
Yes, we cannot schedule your initial evaluation without having a copy on hand of a prescription for therapy signed and dated from your physician. Your initial prescription is only valid for 30 days.
What insurances do you accept?
We accept most insurance plans, our team of registrars and insurance specialist at our Central Scheduling office will verify coverage so you'll know exactly what services are covered by your plan.
Do I pay my co-pay at time of service?
Yes, we can collect your co-pay over the phone when you schedule your initial evaluation. We also collect co-pays at our locations on your date of service for continuing therapy appointments.
Is there financial assistance available?
Yes, there are financial counselors in our Patient Accounts office who can see if you qualify for any assistance. Ask your scheduler for more information when you are scheduling your initial eval.
What should I bring with me when I come for my evaluation?
You will need to bring a copy of your insurance card, a photo ID and a method of payment for your co-pay if you didn't already pay that over the phone when you scheduled your initial evaluation. Wear loose fitting, comfortable clothes so they don't hinder movement during therapy. Please also bring your list of current medications. If you were able to print off our medical history form, please bring that with you as well. It can be found here (add link to med history form).
How long will my appointment last?
Your initial evaluation will last 60-90 minutes, depending on your specific appointment. Your scheduler can confirm the exact length of your appointment when you are scheduling your appointment.
Where should I park?
At our Pequot location, there is free Valet services for all patients. The Valets are located at the main entrance. You can also park on your own if you so choose. Our Waterford and Flanders locations have plenty of convenient parking in front of each office.
