The Department of Medical Staff Services provides assistance to Medical Staff and Health Professional Affiliate Staff. Please contact us with questions or assistance with any of the following:
- New Medical Staff or Health Professional Affiliate Staff applications
- Reappointment of Medical Staff or Health Professional Affiliate Staff
- Addition or change of privileges for current Medical Staff and Health Professional Affiliate Staff
- Request a leave of absence or return from a leave of absence
- Notification of a change in practice location, address, malpractice insurance carrier or changes to any contact information
- Medical Staff dues
- To enroll as a volunteer in the Connecticut Statewide Emergency Credentialing Program
