Community Health Needs Assessment

Understanding the current health status of the community is important in order to identify priorities for future planning and funding, existing strengths and assets upon which to build, and areas for further collaboration across organizations, institutions, and community groups.

The Health Improvement Collaborative of Southeastern CT (HIC) — a coalition of community members, health care providers, local public health, tribal representatives, higher education, and numerous non-profit organizations serving the region — engaged in a process to assess the community's health needs as part of a larger goal of measurably improving the health and wellbeing of residents.

Lawrence + Memorial Hospital, as part of the HIC, recently completed the 2022 community health needs assessment process. This included the development of a comprehensive Community Health Improvement Plan (CHIP) by the hospital to address the major unmet needs in the region.
