Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Annette Greene, information associate, Emergency Department, was recognized for practicing a questioning attitude and advocating for patient care. A patient discharged from the ED with a prescription for antibiotics was unable to pick them up because the pharmacy was closed. While talking to the patient, Greene noticed that his speech was altered. When she asked if he was having any difficulty swallowing, the patient replied that his tongue did feel unusual and swollen. She urged him to return to the ED as soon as possible, where it was determined the patient needed emergency respiratory assistance.
 
Gene Thompson, surgical technologist, Saint Raphael Campus OR, was recognized for attention to detail, practicing a questioning attitude and stopping the line to speak up for safety while assisting during a spinal procedure. The surgical team took an image to evaluate the hardware placed during surgery, noted that everything was satisfactory and began to close up. Thompson’s internal smoke detector went off, and he asked if the screw caps had been removed from the screws. The surgical team accepted his cross check, removed the caps before closing the incision and expressed gratitude for Thompson’s use of CHAMP behaviors.
