Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Sara Rapuano, Emergency Department technician, Saint Raphael Campus, was recognized for attention to detail and escalating her concerns while assisting a patient who arrived at the ED with her infant. The patient called a family member to pick up the infant. While observing the interaction between the patient and visitor, Rapuano sensed that something was wrong. She escorted the visitor with the infant to her car and saw there was no car seat in the vehicle. Rapuano immediately contacted Social Work staff, who ensured the infant was safely returned home. 

Richard Standish, RN, SRC Operating Room, was recognized for attention to detail. While prepping a patient for surgery, he noted that the patient had been marked on both hips. Standish stopped the line and escalated his concerns to the surgical team, who validated and verified the correct surgery site, avoiding a potential wrong-site surgery.
