Great Catch winners help YNHH become a high reliability organization

great catch

Ashia Aguilar, Louise Desiderio

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Inpatient

Ashia Aguilar, patient care associate, Medicine (EP 4-7), was recognized for practicing a questioning attitude and 200% accountability when she heard a patient coughing uncontrollably in another room. Aguilar stopped what she was doing to check on the patient and found her in distress. She escalated her concern to the nurse, who evaluated the patient and called the Rapid Response Team. The patient required additional critical resources, so she was transferred to a higher level of care.

Outpatient

Louise Desiderio, patient financial access facilitator, Devine Street, North Haven, was recognized for attention to detail using STAR (Stop, Think, Act, Review) and sharing her concerns when she answered a phone call from a patient who complained of being short of breath. Desiderio was advised to schedule an evaluation appointment for the patient, but after talking with the patient again, Desiderio escalated her concern. After talking with the patient on the phone, a nurse called 911 and stayed on the line with the patient until help arrived.
