YNHCH recognized for patient safety

Solutions for Patient Safety (SPS) recently recognized Yale New Haven Children’s Hospital for its exemplary culture of safety.

SPS is a national network of 140-plus children’s hospitals that works to eliminate serious harm across all children’s hospitals and helps individual hospitals progress toward zero harm. 

“Congratulations to the team at Yale New Haven Children’s Hospital for achieving distinction in the annual SPS Shine Report for their culture of safety,” said Rebecca Ciaburri, RN, system director, Quality, Safety and Program Development, YNHCH. “This is a much-deserved recognition for our staff, who constantly elevate our standards of safety in the face of an ever-changing healthcare landscape.” 

SPS recognized several YNHCH safety practices, including:

  • An organizational safety brief (Morning Safety Report) that allows for quick escalation of problems and accountability for safety events
  • Error prevention training for all new hires, along with ongoing error prevention education
  • A dedicated Quality and Safety Team responsible for safety event review and cause analysis
  • A safety coach program that includes monthly meetings with frontline staff

SPS also noted YNHCH’s focus on recognizing achievements in harm reduction and promoting the Great Catch program, which honors staff for using High Reliability Organization behaviors.
