Great Catch winners help YNHH become a high reliability organization

Congratulations to these Great Catch winners for modeling HRO safety behaviors and taking action to improve patient safety.

Rosa Bolt, environmental services associate, EP 9-5, was recognized for attention to detail, practicing a questioning attitude and escalating concerns. Bolt overheard a conversation between a patient and family member that included comments which set off her “internal smoke detector.” She alerted the nurse, who discovered that the patient had illegal drugs. Bolt’s use of CHAMP behaviors prevented potential harm to the patient.

Loida Cayetano, patient care associate, Saint Raphael Campus PACU, was recognized for attention to detail and practicing a questioning attitude. While helping a patient get settled in her room before surgery, Cayetano became concerned when the patient complained of feeling “shaky.” Upon questioning, Cayetano discovered the patient had diabetes. She quickly retrieved a glucometer and alerted the nurse, who discovered that the patient’s glucose level was dangerously high. Because the patient was to receive local anesthesia versus general, a glucose check was not part of the usual pre-op protocol. The surgery was postponed and the patient was transferred to the ED.
