Outpatient

Amjad Raza, sonographer, Heart and Vascular Center, was recognized for attention to detail while performing an echocardiogram on a patient who was 34 weeks pregnant. The preliminary report identified an issue that triggered Raza’s internal smoke detector.

He escalated his concerns to the radiologist, who performed a cross check. The finding was confirmed and the care team was able to immediately develop an appropriate plan of care for both mother and baby.

Inpatient

Sherry Staggers, patient financial access specialist, Emergency Department, was recognized for attention to detail and practicing a questioning attitude after completing a patient registration. The 86-year-old patient was supposed to have an emergency catheterization due to chest pain. Staggers saw that the patient’s medical record had been confused with that of another patient who had the same first and last name but a different birth date. She escalated her concerns and the unnecessary procedure was canceled for the 86-year-old patient.
