Admissions
On your day of admission, please arrive on time. If you are already registered, you may need to sign some final paperwork. At that time, an ID bracelet will be placed on your wrist. If it's ever removed, simply ask for a replacement. Papers/identification that you might need at admission:
- Medical insurance cards
- Picture ID
- A list of all medications that you are currently taking
- If you have an advance directive, such as a living will or a health care agent, please bring a copy with you. A living will cannot be honored unless it is attached to your chart. Find out more about Advance Directives.
If You Have a Hearing Problem
To ensure effective communication with patients and companions who are deaf or hard of hearing, we provide appropriate auxiliary aids and services free of charge, such as:
- Sign language and oral interpreters
- TTY's
- Note takers
- Computer-assisted real time transcription services
- Written materials
- Telephone handset amplifiers and telephones compatible with hearing aids
- Assistive listening devices and systems
- Closed caption decoders
- Open- and closed-captioning of most Hospital programs
