Community Health Needs Assessment
Understanding the current health status of the community is important in order to identify priorities for future planning and funding, the existing strengths and assets on which to build upon, and areas for further collaboration across organizations, institutions, and community groups. The Southeastern CT Health Improvement Collaborative — a coalition of health care providers, local public health, federally qualified health centers, tribal representatives, higher education, and numerous non-profit organizations serving the region — led a process to assess the health needs of community, as part of a larger goal of measurably improving the health and wellbeing of its residents.
Lawrence + Memorial Hospital and Ledge Light Health District (LLHD), as part of the Southeastern CT Health Improvement Collaborative, recently completed the 2019 community health needs assessment process. This included the development of a comprehensive Community Health Improvement Plan to address the major unmet needs in the region.
