Community Health Needs Assessment
Guided by the Southeastern CT Health Improvement Collaborative — a coalition of health care providers, local public health, federally qualified health centers, tribal representatives, higher education, and numerous non-profit organizations serving the region — Lawrence + Memorial Hospital and Ledge Light Health District (LLHD) considered data from primary and secondary sources to identify and elucidate the leading health indicators for the region included in a Community Health Needs Assessment. Accompanying this report s a Community Health Improvement Plan (CHIP) to address the Community Health Assessment findings.
