Outcomes Reporting

Outcome Data

Quality Rehabilitation Services at Lawrence + Memorial Hospital

We track and document the success and effectiveness of our programs through the use of the following independent national data collections systems.

Commission for Accreditation of Rehabilitation Facilities

CARF reviews and grants accreditation services nationally and internationally on request of a facility or program. Their standards are rigorous, so those services that meet them are among the best available. CARF’s focus is on ensuring the services you use meet your needs for quality and the best possible outcomes.

Uniform Data Systems for Medical Rehabilitation

One of the tools we utilize to measure our quality is UDSmr. This tool allows us to measure ourselves against the region and the nation. In doing this we are able to see how we “measure up”. This national outcome system supplies us with necessary information so we can make informed decision, identify opportunities for improvement and provide the best care possible.

The FIM System

The FIM System® is an outcomes management program. The FIM System® enables providers and programs to document the severity of patient disability and the results of medical rehabilitation and establishes a common measure for the comparison of rehabilitation outcomes.

The FIM System® provides an established means of collecting rehabilitation data in a consistent manner. It allows clinicians to follow changes in the functional status of their patients from the start of rehabilitative care through discharge and follow-up.

IT HealthTrack Inc.

IT HealthTrack was established to promote quality in healthcare by providing facility administrators with valid and reliable data collection and management services for measurement and improvement of followup outcomes related to patient functional performance.
